If you take insulin injections
Medicare will cover 300 test strips & lancets per 3 months.
If use more than 300 strips per 3 months,
Medicare requires a copy of a current log book
and documentation from your doctor.

If you do not take insulin injection
Medicare allows 100 test strips per 3 months.
If you use more than 100 test strips per 3 months,
Medicare requires a copy of a current log book or sheet,
and documentation from your doctor.
